On the whole many loop holes exist in the organizational structure and there is a needed to initiate a change process within an organization. The change process that is needed to be launched must incorporate all the necessary steps require to ensure patient’s safety. To do so the communication mechanisms existing in the hospital will have to strengthened. In addition to this the change process must be aimed to promote the trend of documentation because documentation will help the organization to take a more evidence based approach rather than depending merely upon generalized facts about the state of patients (Holmes, 1998, July). The communication level is needed to be enhanced not only inside the hospital but there is a need to devise a wide and flexible communication plan for the communication that takes place with parties outside the hospital e.g. families of patients. Some structural changes are also required to be initiated and among those the most important one is related to duties of nurses. Duties must be designed in a way that no patient is left unattended particularly those who are totally dependent upon others (Michelson & Suchitra, 2004).
